Question exceptions are caused by students filling more responses than the answer key contains, which results in a score of 'zero' for that question (meaning that all the test sheets listed on the question exceptions have been given a 'zero' for that question). 

These are called as exceptions, because Akindi is unable to tell the difference between a filled bubble and an 'X'ed-out-filled-bubble.
